Mortality Reviews

The Columbus Organization conducts mortality reviews under contracts with multiple states and the District of Columbia. We provide mortality reviews that are non-intrusive and non-punitive.  We partner with agencies and facilities in a collegial manner because our focus is on helping make substantive changes and improve the quality of life for the individuals served. 

We deliver detailed positive findings along with any potential areas of concern and offer recommendations so that you can do what you do best.

The Columbus Organization can provide mortality reviews or investigation services to meet your particular needs:

  • Mortality investigation and reviews
  • Trend analysis of internal mortality investigations and reviews
  • Quality reviews of internal death investigations
  • On-site reviews of the immediate circumstance surrounding the death, typically within three working days of the notification of the death
  • Assistance with completing internal mortality investigations

The Columbus Organization has more than 30 years of experience conducting Mortality Reviews and Investigations for individuals with intellectual and developmental disabilities in many states including: California, New Mexico, Tennessee, Nebraska, District of Columbia, Georgia, New Jersey, Connecticut, Iowa, Louisiana, Missouri, Washington, and Texas. 

Many of The Columbus Organization’s mortality review engagements have been with agencies engaged in litigation by the U.S. Department of Justice, state Disability Rights organizations and other attorneys experienced in civil rights law.  The quality of The Columbus Organization’s mortality review processes has often been recognized in lawsuits, such as, those in D.C. where The Columbus Organization has been retained since 2002. The Columbus Organization team of professionals includes physicians, nurses, trained investigators and data analysts with intellectual and developmental disability experience.

The September 2015 Report of the Independent Reviewer in the US vs. Georgia case noted:

“The Columbus process is both positive and rigorous and produces in-depth, independent reviews to highlight and address concerns in the State’s service-delivery system.
