The Columbus Organization conducts mortality reviews under contracts with multiple states and the District of Columbia. We provide mortality reviews that are non-intrusive and non-punitive.  We partner with agencies and facilities in a collegial manner because our focus is on helping make substantive changes and improve the quality of life for the individuals served.

The September 2015 Report of the Independent Reviewer in the US vs. Georgia case noted:

“The Columbus process is both positive and rigorous and produces in-depth, independent reviews to highlight and address concerns in the State’s service-delivery system. The reports are well-written and contain findings, more exact and accurate causes of death, positive findings, concerns associated with the death (if any), and individual and systemic recommendations to help avoid preventable illness and death going forward.”
