Case Management in New Mexico

The Columbus Organization provides person-centered and choice-based support coordination services modeled on the principles of self-determination. Our focus is on providing individuals with intellectual and developmental disabilities the opportunity to lead a self-directed life with improved employability, relationships, and ability to make positive life decisions. We are committed to improving support for the people we serve through ongoing education & training, community awareness programs, advocacy, and thought leadership.

Case Management Service is provided to individuals receiving Supports Waiver Program (SWP).

The Columbus Organization also provides Case Management services to individuals enrolled Developmental Disabilities Waiver Program (DD).
