The Columbus Organization is now providing Case Management for individuals and their families throughout Minnesota.
The Columbus Organization is approved by the Department of Human Services to provide case management to individuals on the Developmental Disabilities Waiver (DD) and Non-Waiver (Rule 185 and VADD) programs.
Our process is responsive to changing needs and ensures services and supports based on the team working together. We pay close attention to outcomes and the achievement of specific, person-centered, measurable goals to determine the level and type of service needed to maintain successes over the long term. Your case manager is a member of the team who works in close collaboration with your family and all other team members.

MN Case Manager, Jorie C.:
Jorie is a graduate from Indiana University, Bloomington, with a BA in Psychology, and brings over 18 plus years of experience, ensuring quality person-centered program and service provision for individuals with intellectual and developmental disabilities; her background also includes diverse leadership roles held across non-profit organizations, managed care and state governments (Indiana State Department of Health & Minnesota Department of Human Services).
Before moving to Minnesota in 2007, she gained extensive experience on the provider side of service provision managing day program settings, residential settings, including DD case management and community integration.
On the quality and regulatory side of service provision, she also brings strong regulatory and compliance experience as a Medical Surveyor with the Indiana State Department of Health, surveying state and federal regulations impacting state licensure status for state hospitals and residential settings across Indiana.
Upon moving to Minnesota she continued to gain extensive leadership experience in program management/case management at Fraser, compliance experience as Manager of Healthcare Improvement and Compliance at Blue Cross Blue Shield of MN and worked the next 10 years in Disability Services and Managed Care at the MN Department of Human Services focusing on key state wide projects such as MnCHOICES assessment, care plan audit procedures and operations impacting health policy and people in receipt of home and community based programs.
Serving as a Case Manager for Minnesota (Hennepin County), she is looking forward to supporting the needs of individuals and families and attributing her strengths in the areas of case management, person-centered planning, positive behavior supports, and quality/compliance oversight.
In her free time, she enjoys time with her husband and two schnauzers, traveling, yoga and exploring a new interest with vegan recipes and gardening.
