Indiana Case Management Team

Jacqueline Pulling, Executive Director. Ms. Pulling has worked in Case Management since 2000. In this role, she has managed large caseloads, advocated for her clients, facilitated annual interdisciplinary team meetings, and completed high-risk assessments. Ms. Pulling received her Bachelor's degree in Psychology from Indiana University South Bend. Ms. Pulling serves the Upper Northern Indiana (South Bend) region..

Katie Sloan, Regional Director. Ms. Sloan has worked in Waiver Case Management since 2007.  Prior to working in DD waiver case management, she worked with the area agency on aging with the AD waiver.  She is experienced in working with individuals with developmental disabilities; families, and children with behavioral issues. Ms. Sloan has a Bachelor's degree in Social work from Ball State University. Ms. Sloan serves the East Central/Central Indiana (Muncie, Kokomo) region.

Linda Chase - Team Leader.  Ms. Chase has worked in Case Management for over 15 years.  She is experienced in serving adults and children with physical and intellectual disabilities, as well as clients with high behavioral needs.  Ms. Chase is experienced in writing case plans within budgetary guidelines, facilitating team meetings and work closely with families and providers.She received her Bachelor's Degree in Social Work from the University of Illinois-Chicago.  Ms. Chase is serving the Central Indiana (Indianapolis) region.

Summer Marshall, Team Leader.  Ms. Marshall has worked in human services over ten years. Her diversified professional experience includes working as a waiver case manager. She is experienced with leading teams, developing relationships with families and service providers and helping individuals access all available community resources.  Ms. Marshall received her Bachelor's degree in Business Administration from Indiana University East. Ms. Marshall serves the South East Indiana region.

Anabel Quiroz-Aguilar, Team Leader.  Anabel Quiroz-Aguilar is a bi-lingual case manager. Anabel has worked in the social service field since 2008 serving families and individuals.  Her professional experience includes working as a waiver case manager. She's experienced in monitoring plans with service providers, managing large caseloads, and working closely with families to understand their individual needs. Anabel received her Bachelor's degree in Youth, Adult, and Family Services from Purdue University. Anabel serves Northwest Indiana, Lake County region

Nikki Furry, Team Leader.  Nikki Furry has over 23 years experience as a Case Manager working with families, children and community based services.  She has worked for the Department of Child Services (DCS), Licensed foster care and adoption agency, as well as, a Director of a non-for profit agency working for prevention of child abuse and neglect.  Nikki has a passion for connecting families to local resources and community supports.   She has extensive knowledge for local service providers in Delaware County and surrounding areas.  Nikki is knowledgeable with developing and facilitating team meeting, case plan and treatment plans.  Nikki has a Master of Arts Degree (M.A.) and a Bachelor of Social Work (BSW) both from Ball State University. Nikki serves the East Central Indiana area.

LaVada Brooks, Case Manager. Ms. Brooks has worked in Case Management since 2006 as a First Steps Service Coordinator and as a Waiver Case Manager. She is experienced in working with high-risk cases as well as facilitating both annual interdisciplinary team meetings and assessments. She is knowledgeable with State waiver services, assuring all Provider State and Federal guidelines are complied with, and site monitoring. Ms. Brooks is a CPR/FA instructor and received her Bachelor's degrees in Biology and Secondary Education from Indiana University Southeast. Ms. Brooks serves the Southern Indiana area..

Patricia Herrera, Case Manager. Ms. Herrera has worked in Case Management since 2001. Her experience includes writing financial case plans for Medicaid Waiver, monitoring plans with service providers, managing a large caseload, and working closely with families. She received her Bachelor's degree in Sociology and Psychology from Calumet College in Indiana and her Master's in Health Care Management from the University of Phoenix. Ms. Herrera serves the Upper Northwest Indiana (Gary) region.

Robin Kegebein, Case Manager. Ms. Kegebein has worked as a Case Manager since 2003. With over 20 years' experience working in the developmental disabilities field, she has gained knowledge and understanding of waiver opportunities. Ms. Kegebein has assisted individuals with a wide range of needs. Ms. Kegebein received her Bachelor's degree in Secondary Education from Indiana University South Bend. Ms. Kegebein is currently serves the Upper Northern Indiana (LaPorte) region.

Jennifer Baldwin, Case Manager. Ms. Baldwin has worked with people with disabilities for over 20 years, in various capacities from direct care to facilitating and advocating for them. Ms. Baldwin has experience working with children and adults with a wide range of abilities and needs. She has a Bachelor's degree in Social Work from Eastern Mennonite University. Ms Baldwin serves the Upper Northern (South Bend & Elkhart) and Northeast Indiana (Fort Wayne) region.

Dana Houston, Case Manager. Ms. Houston has worked in Case Management since 2010 and has over 20 years of experience in the social service field.  Her experience includes strongly advocating for individuals with disabilities, writing case plans, facilitating team meetings and working closely with families.  She received her Bachelor's degree in Sociology from Purdue University Calumet.  Ms. Houston serves the Upper Northwest Indiana/Lake County areas.

Adriana Torres, Case Manager.  Adriana Torres has been in case management since 2002. Adrianna has been successful at developing plans to address Individual and Family needs and locate appropriate community resources to assist. Adriana has participating in Community Outreach to educate diverse populations. Adriana graduated in 2001 with a Bachelor of Arts, Major in Spanish with a minor in Secondary Education from Martin University. She is currently serving the Marion county area.

Shauna Smith, Case Manager.  Ms. Smith has been working in the social services field since 2007.  She has experience working with individuals with behavior, emotional, mental health disorders and a variety of learning disabilities.   She has worked as a therapist utilizing a wide variety of research-based intervention skills including crisis intervention, parent education, skill building and cognitive/behavior therapy.  Ms. Smith has developed relationships with local community agencies and works to 

Jennifer England, Case Manager.  Ms. England has worked in the field of case management since 2000, the last 7 years working with individuals who are in the Medicaid Waiver program.  Ms. England enjoys building rapport with her clients and their families while acting as their advocate and supporting them as they learn to navigate through the programs and services available to them. Ms. England received her Bachelor’s degree in child development and family studies from Purdue University. Ms. England lives in Johnson County and serves the central Indiana area.

Natalie Shobe, Case Manager.  Ms. Shobe has worked in the field of Developmental & Intellectual Disabilities for over 13 years.  She has experience working in residential programing, day programing, employment, as well as large ICFMR and facility based supports.  Ms. Shobe works diligently to advocate for individuals and their families as well as the ability to establish strong relationships.  She has extensive experience in creating and assisting to implement individual support plans.  Ms. Shobe received a Bachelor’s of Science in Criminology from Indiana State University in 2002. She is currently serving the Marion County and surrounding areas.

Nicole Bell, Case Manager.  Ms. Bell has worked in case management since 2008. She has personal and professional experience working with children and adults with a wide range of abilities and needs. Ms. Bell is knowledgeable with programs and services available under the waiver program as well as providers in the community. She is successful at facilitating team meetings to ensure that services are being provided accordingly and most beneficially to her clients. Ms. Bell received her Bachelor's degree in psychology from Indiana State University. She serves west central Indiana.

Danielle Taylor, Case Manager.  Mrs. Taylor has been in Human Services since 2007.  It is her experience to advocate for her consumers and to make sure that they are receiving the best possible services available to them.  Prior to working in Case Management, Mrs. Taylor worked in the school system, worked in the areas of direct care support and fostered a child.  Organization is a strong skill as Mrs. Taylor has prior business management experience.  She received her Bachelor's Degree in Human Development and Family Studies with a Minor in Sociology from Indiana University, Bloomington.  Mrs. Taylor serves Marion County and surrounding counties.

Heather Royse, Case Manager.  Ms. Royse began her career in 2013 as an A&D Waiver Case Manager at a local AAA. During her time there she gained knowledge of community resources, advocated on behalf of clients with complex medical issues, and worked closely with service providers and families to ensure client needs were met. Before her time as a case manager she volunteered at local nursing facilities in the Muncie region. She received a Bachelor’s Degree in Social Work from Ball State University in 2013. Ms. Royse serves the East Central Indiana region.

Juana King, Case Manager.  Ms. King has worked with people with disabilities for over 10 years: advocating on behalf of their needs, supporting individuals to increase independent living skills, and managing and coordinating care plans.  Ms. King was a Program Coordinator for an Adult Day Program. Ms. King also provided direct care for individuals with disabilities while working in Residential services. Ms. King obtained her Bachelor's degree in Sociology at Martin University in Indianapolis, Indiana. Ms. King will be providing case management services to Marion County and the surrounding counties.

 

Adrienne Jessie, Case Manager.  Ms. Jessie has worked in Case Management since 2014.   In this role, she has worked with her clients to make sure their needs were met as she has worked as an advocate for them.  She has also facilitated quarterly meetings with her clients and their teams.  Ms. Jessie also worked has a developmental therapist for 7 years.   Ms. Jessie received her Bachelor's degree in Early Childhood and Primary Education from Ball State University. Ms. Jessie is serving the Indianapolis and Central Indiana region.

Nichole Mendenhall, Case Manager.  Nichole Mendenhall has served in the field of Community, home and Supported Group Living for the last 14 years. She has a psychology degree from the University of Phoenix.   Mrs. Mendenhall has worked with both adults and children who need additional supports in their communities to live as independently as possible.  Mrs. Mendenhall currently serves the Central and Western regions including the counties of Vigo, Sullivan, Knox, Greene, Owen, Clay and Putnam.

Kelsey Isenberg, Case Manager.  Ms. Isenberg has been working as a case manager since 2012 in youth residential treatment facilities and as a Wraparound Facilitator for Marion County. She has worked with youth and families of various backgrounds and cultures, advocating and building community supports to prevent the need for residential treatment, or helping prepare for re-entry into the community from residential settings. Ms. Isenberg earned her Bachelor’s degree in Criminology from Butler University and is serving Hamilton and surrounding countie

Esteban Hillyard, Case Manager.  Esteban Hillyard has worked in the field of Developmental and Intellectual disabilities for 4 years. He graduated Indiana University - Purdue University with a Bachelor's degree in Psychology. Mr. Hillyard has aided individuals of all ages live as independently as possible under their particular circumstances. His experience working alongside such a diverse and extensive client base has provided him with a compassion for, and understanding of the field, and has allowed him to be able to successfully and quickly adapt to situations.  Mr. Hillyard will be serving Allen and surrounding counties.

Amanda Albertson, Case Manager.  Ms. Albertson is a Licensed Social Worker in the State of Indiana.  She obtained her Bachelor of Science in Social Work from the State of Minnesota.  She moved to Indiana in 1999 and started her career as a Case Manager with the Area Agency on Aging and Disabled located in Terre Haute, IN.  Her professional experience includes Waiver Case Management, long term care, hospice, and health care in West Central and Southern Indiana.  She has primarily assisted the elderly and disabled population in living the best quality of life as independently as possible. Amanda is knowledgeable in American Sign Language and hearing impairment. She brings both personal and professional experience to the individuals she serves

Denise Holba, Case Manager.  Ms. Holba has worked in human services since 2010. Her diversified professional experience includes working in Research with our Armed Services and Parent education in early childhood development. As a Case Manager, she is experienced with leading groups, developing relationships, and helping individuals reach their goals.  Ms. Holba received her Bachelor's degree in Psychology from Michigan State University and MSW from University of Illinois-Chicago. 

Ermin Purcar, Case Manager.  Mr. Pucar has provided social work services in Indiana since 2010. He has worked with individuals of all ages and abilities. Mr. Pucar has been successful at maintaining ongoing communication with individuals, families, and providers. He uses resources available to him and his knowledge of the Medicaid Waiver Program to ensure the needs of the individuals he serves are met. Mr. Pucar received his Bachelor's degree in Criminal Justice from Indiana University South Bend. Mr. Pucar is serving the Upper Northern Indiana (South Bend/Elkhart) Region.

Doribell Nelson, Case Manager.  Ms. Nelson is a Spanish Speaking Case Manager. She has provided quality services to Spanish Speaking families throughout Northwest Indiana for more than six years. Ms. Nelson's previous experience includes working for a non-profit child development organization. She is successful with wroking in multicultural environments and multidisciplinary teams. Ms. Nelson has a Bachelor Degree in International Relations. She serves the upper Northwest Indiana/Lake County Region. 

Fabiola Macias, Case Manager.  Ms. Macias is a bilingual case manager. She graduated from Indiana University Purdue University Fort Wayne in 2014 with a Bachelor's Degree in Public Policy.  In 2015 she obtained her second Bachelor's Degree in Spanish. Her experience in case management has been with Amani Family Services (2015) working with immigrants  and refugee families by providing one-on-one in home case management by promoting safety, encouraging personal growth, and to assist in fostering a spirit of belonging within the community.  Ms. Macias was also was a Spanish interpreter for a mental health counselor and took a training on medical interpretation.  Ms. Macias will be serving Allen and surrounding counties.

Erika Birchler, Case Manager.  Ms. Birchler  joined The Columbus Organization in November of 2016.  Prior to working with the Columbus Organization she has provided case management services to families for five years in  Evansville, IN. She also worked in the medical field for 7 years. She is experienced in working with individuals of all ages with physical and intellectual disabilities, as well as clients with high behavioral needs. Ms. Birchler received her bachelor’s degree in Psychology from the University of Southern Indiana. Ms. Birchler serves the Southwestern Indiana region.

Laura Todd, Case Manager.  Ms. Todd joined The Columbus Organization starting in April of 2017.  Ms. Todd lives in Madison, IN, and will be serving the Southeast region of the state.  She has an Associate of Science degree in Nursing and a Bachelor of Science degree in Nursing.  Ms. Todd has almost five years of Nursing experience.  She looks forward to a rewarding career in Case Management. Some aspects of this job that she will enjoy are working with and helping clients and using her medical background to help clients understand their condition and available services. Ms. Todd serves the South East Indiana region.

Jessica Dudley, Case Manager.  Ms. Dudley has worked in Waiver Case Management since 2007 and in the social service field since 2001.  She is experienced in working with individuals with developmental disabilities, families, and children with behavior issues.  She is knowledgeable with State Waiver Services and community resources.  Ms. Dudley is experienced with facilitating meetings, working with service providers and working with Medicaid cases.  Ms. Dudley has a bachelor’s degree in Social Work from Ball State University. Jessica serves Muncie and surrounding counties.

Nuria "Rhea" Romero, Case Manager.  Ms. Romero is a Bi-lingual case manager. She has worked with individuals with developmental and intellectual disabilities since 2011. Ms. Romero has a vast knowledge of wavier services with several years of experience working in management for a waiver service provider.  She has also worked with the Department of Child Services regarding the care of individuals that have developmental and intellectual disabilities. Ms. Romero feels passionate about ensuring that her client’s needs are met and the families concerns are heard. She received her Bachelor’s degree in Pre-Law from Calumet College of St. Joseph. Ms. Romero serves (St. Joseph/ Elkhart) and surrounding                                   counties.

Julie Varvel, Case Manager.  Ms. Varvel joined The Columbus Organization team in January of 2017.  She has 12 years of experience in the field of developmental disabilities.  Ms. Varvel began her career by working directly with individuals in their homes.  She has supervised both supported living settings and group home settings.  Ms. Varvel enjoys working with individual’s to assist them in becoming more independent. She has worked with individuals of all age groups, with a vast range of disabilities and has experience with Autism and individual’s with high behavioral needs. Ms. Varvel received her bachelor’s degree in Psychology from Indiana University and serves the central Indiana region.

 

Mandy Schmitz, Case Manager.  Ms Schmitz has worked in the Social Services field for over 15 years in a variety of settings and programs.  She has worked as an A&D Waiver Case Manager for the past 7 years, managing an on-going caseload of clients receiving in-home services to ensure their care plans are current and meeting their individual needs.  Much of her experience includes locating community resources and acting as an advocate for her clients.  She has experience in working with diverse populations.  Ms Schmitz received her Bachelors Degree in Social Work from the University of Southern Indiana.  Ms Schmitz serves the Southwestern Indiana Region.
 

 

 
Lisa Worley, Case Manager.  Ms. Worley has been working with children of emotional, intellectual and behavioral needs since 2011 primarily in the school system.  In 2015, she joined First Steps of Central Indiana as an Intake Coordinator, where she was able to assist many children is getting services instated and also gained case management experience in working with many central Indiana families.    Ms. Worley enjoys working with clients to help them benefit in the best way to the services available to them.  Ms. Worley has a Business degree from Ball State University with a minor in Psychology.  She will be serving northern Marion and Hamilton County.
 

 

Irene Gonzalez, Case Manager.   Ms. Gonzalez has been in case management since 2013. She has experience working with families, children, and community based services as well as home based services as an individual contractor for The Department of Child Services for the State of Indiana.  She is experienced in serving a diverse population in Indianapolis. Ms. Gonzalez enjoys being able to make a difference in the individual’s lives she gets the chance to work with.  She received her Bachelor's Degree in Social Work from the University of Indianapolis.  Ms. Gonzalez is serving the Central Indiana (Indianapolis) region.
 
 
 
 
Patricia Reed, Case Manager. Ms. Reed is a Bilingual Case Manager in the Central Indiana region. Ms. Reed received a Bachelor of Science Degree in Chemistry from VA Commonwealth University.  She brings with her 15 years of professional experience in working with individuals with disabilities. Before joining Columbus, Ms. Reed worked as a Bilingual Parent Liaison for About Special Kids for over 11 years. As a Parent Liaison, she assisted families and professionals in navigating multiple programs and services to improve the lives of children with special needs.  Ms. Reed was a member of Answers for Autism board of directors for 15 years, seven of those years she served as President. As well as her professional experience she also brings over 20 years of experience as the parent of an individual on the autism spectrum.
 

 

Megan Letourneau, Case Manager.   Ms. Letourneau has worked in the social services field since 1998. She earned her Bachelor’s and Masters of Social Work from Indiana University.  Megan enjoys building rapport with consumers to learn who they are and to determine what it is they need, then working with them towards a solution. Megan is strengths based, problem-solving advocate for clients. Megan lives on the West side of Indy and is accepting clients in Marion, Morgan, Hendricks, Putnam and Boone Counties.
 
 
 
 
 
Tara Miller, Case Manager.   Ms. Miller is a graduate of Franklin College.  After college, she worked in special education assisting middle school students.  In 2002, she moved to Los Angeles, CA, where she continued to work in special education, with preschool-aged children.  She transitioned from working in the school-based setting to working in residential homes for adults with developmental disabilities.  Eventually, she became a Program Director for a site-based day program serving adults with developmental disabilities in Los Angeles.  Tara relocated back to Indiana in 2016, and has worked with the Columbus Organization for a year.
 

 

Rocio Castellanos, Case Manager.  Ms. Castellanos is a bilingual case manager. She has worked in case management since June 2013. Prior to working as a Waiver Case Manager, she worked as a case manager for a nonprofit foster care organization. Ms. Castellanos received her Bachelors in Psychology from Governors State University. She serves the Northwest Indiana, Lake and Porter County Region.
 
 
 
 
 
 
Nicole Fitzsimmons, Case Manager.  Ms. Fitzsimmons has worked with individuals with developmental and intellectual disabilities since 2011. She has a vast knowledge of wavier services with several years of experience working in management for a waiver service provider. Ms. Fitzsimmons feels passionate about ensuring that her client’s needs are met and the families concerns are heard. She received her Bachelor’s degree in in Business Administration from University of Wisconsin- Whitewater.
 
 
 
 
 
 
Jana Cocquyt, Case Manager.  Ms. Cocquyt has worked in the Social Services area for many years.  She has experience in treatment facilities, outreach, school systems, and early intervention. She has experience with both working with children and families. Most recently, she worked for First Steps as an Intake Coordinator the last 4 years.  In this position she assisted families in getting the necessary services for their child and connecting families with other outreach programs they may need .Ms. Cocquyt graduated with a Bachelor degree from Ball State University.  She will be serving Hamilton and other surrounding counties.
 
 
Lastashia Flannery, Case Manager.  Ms. Flannery has been working in the Human Service Field for over 10 years in many capacities. Ms. Flannery started as a direct support staff after high school and has worked with youth and families of various backgrounds and cultures. Ms. Flannery earned her Bachelor’s degree in Social Work from the University of Indianapolis and is serving Marion and surrounding counties.
